Schedule of Availability of Service: Monday to Friday (8:00 AM to 5:00 PM without Noon Break)
Who may avail of the Service: Real Property Owners

Requirements: 1 copy of the following:

TRANSFER OF OWNERSHIP

  • Transfer Certificate of Title
  • Latest Realty Tax Receipt or Tax Clearance
  • Certified True Copy of Certificate Authorizing Registration (CAR)
  • Transfer Tax Receipt or Certification
  • Deed of Conveyance (Deed of Sale, Donation, Waiver of Rights, Sheriff’s Sale, Extrajudicial Settlement of Estate, etc.)

NEW DECLARATION OF BUILDING & MACHINERY

  • Initial Tax Declaration from Municipal Assessor’s Office (MAO)
  • Sworn Statement duly prepare and signed by Owner
  • Field Appraisal and Assessment Sheet (FAAS)
  • Bill of Materials and Occupancy Permit (for Building)
  • Receipt of Purchase or any proof showing Acquisition Cost (for Machinery)

CORRECTION (Spelling of Name, Property Identification No., Barangay, Area, Lot No. etc.)

  • Transfer Certificate of Title
  • Latest Realty Tax Receipt or Tax Clearance

Duration: 3 Working days (24 hrs.) for maximum of 10 Tax Declarations

How to Avail of the Service:

Submission and Verification of Documents
1 Clients submit requirements to Local Assessment Operation Officers (LAOO). Local Assessment Operation Officers (LAOO) checks the correctness of the requirements then forward the documents to the Receiving Officer. 6 hours Local Assessment Operation Officers (LAOO) (Romina A. Receiving Officer issues Routing Slip and Order of Payment. 1 minute Computer Operator I (Mary Joanne Manlapaz) PAO 002 & PAO 005
2 Clients fill up Routing Slip (PAO 002) then proceed to the Office of the Provincial Treasurer for the payment of Secretary’s Fee. Secretary’s Fee: P400.00 (P250.00 -Transferof Ownership Fee P150.00 – IT Fee) PAO 005
3 Clients submit requirements to Receiving Officer together with the Routing Slip (PAO 002) and Official Receipt of Secretary’s Fee. Receiving Officer forwards the requirements to the Data Encoder. 1 minute Computer Operator I (Mary Joanne Manlapaz) PAO 002 & Official Receipt of Secretary’s Fee
Preparation of Tax Declaration
Data Encoder encodes/corrects and prints Tax Declaration, affixes initial on it & on Routing Slip then forwards the documents to the Tax Mapper. 8 hours Administrative Aide II (Bookbinder I)/ Administrative Aide III (Bookbinder I)/ Assessment Clerk II/Computer Operator I (Menette Monta/ Lorena Caracas/ Laura Manalaotao/Mary Rose Adlawan) PAO 002
Tax mapper updates the Tax Mapping Control Roll (TMCR), updates/plots of the Title on the GIS Base Map, affixes initial on Tax Declaration and Routing Slip then forwards the documents to the LAOO. 4 hours Tax Mapper IV/ Tax Mapper II/ Tax Mapper i/ Administrative Aide IV (Clerk II)/ Draftsman I/ Clerk III (Eduardo S. Review and Approval of Tax Declaration
LAOO checks the correctness of the data encoded, affixes initial on the tax declaration and on Routing Slip then forwards documents to the Assistant Provincial Assessor. 5 hours Local Assessment Operation Officers (LAOO) (Aireen Penequito/ Romina A. Estrella/ Joanna Marie Eugenio/ Julie Ann Y. Crisenes) PAO 002
Assistant Provincial Assessor affixes initial on Tax Declaration as to correctness and on the Routing Slip then forwards the documents to the Provincial Assessor. 20 minutes Assistant Provincial Assessor (Mabini R. Pulido, Jr.) PAO 002
Provincial Assessor signs the Tax Declaration, affixes initial on Routing Slip then forwards it to the Releasing Officer. 30 minutes Provincial Assessor (Engr. Ricardo C. Herrera) PAO 002
Releasing of Documents of Tax Declaration
4 Clients receive the Tax Declaration. Releasing Officer records the transaction on the Log Book, issues the Tax Declaration, affixes initial on Routing Slip then forwards the Documents and the Routing Slip to the Records Officer for filling. 8 minutes Administrative Aide I (Utility Worker I) (Maria Cristina C. End of the Transaction
Note: The processing of Tax Declaration using Extrajudicial Settlement of Estate is within 5 working days.
