Who may avail the Service: Letter of request for assistance, death certificate of the deceased, Certificate of incumbency from Barangay Chairman, Appointment, Certificate from MLGOO re: service performed in position, Certificate of indigency from MSWD, Other required documents (including: birth certificate, marriage contract, baptismal certificate, barangay certification, original SPS, photocopy of claimant CTC and valid ID if claimant], transmittal from DILG Bataan of compiled documents
