Frontline Service: Issuance of Veterinary Health Certificate
Schedule of Availability of Service: Monday to Friday (8:00 AM to 5:00 PM without Noon Break)
Who may avail of the Service:

Livestock and/or poultry farmers who will transport stocks within and outside the Province of Bataan
Livestock and/poultry middlemen who will transport animals and animal by-products within and outside the Province of Bataan
Pet owners who will transport their pets outside the Province of Bataan

Requirements:

Vaccination Card as Certificate (Companion Animals)
Veterinary Health Certificate (Livestock and Poultry)
African Swine Fever (ASF) Negative Test Result/ASF-Free Certificate (for Hogs) 7.Avian Influenza (AI) Negative Result / Farm Accreditation Certificate (for Poultry)

Duration: 30 minutes

How to Avail of the Service:

		c. For shipment ofcompanion animals, issue veterinary health certificate and shipping permit.
