Frontline Service: Issuance of Veterinary Health Certificate
Schedule of Availability of Service: Monday to Friday (8:00 AM to 5:00 PM without Noon Break)
Who may avail of the Service:

Livestock and/or poultry farmers who will transport stocks within and outside the Province of Bataan
Livestock and/poultry middlemen who will transport animals and animal by-products within and outside the Province of Bataan
Pet owners who will transport their pets outside the Province of Bataan

Requirements:

Vaccination Certificate (Companion Animals)
Veterinary Health Certificate (Livestock and Poultry)
African Swine Fever (ASF) Negative Test Result/ASF-Free Certificate (for Hogs) 7.Avian Influenza (AI) Negative Test Result / Farm Accreditation Certificate (for Poultry)

Duration: 1-2 hours and 15 minutes

How to Avail of the Service:

1	Fill-out the Google Form	a. Fill-out the VHC according to the information provided in the Google Form	30 Minutes	Ms. JobelleDabuet or Ms. Gladys D. Buensuceso	NONE
		b. Send the filled-out VHC and Order of Payment to the client for printing	15 Minutes-1 hour and 30 minutes	Ms. JobelleDabuet or Ms. Gladys D. Buensuceso	NONE	Veterinary Health Certificate Order of Payment
2.	Print the Veterinary Health Certificate and	Duplicate the Official Receipt	10 Minutes	Ms. JobelleDabuet or Ms. Gladys D. Buensuceso	Php 100.00 per Veterinary Health Certificate	Official Receipt
	Order of Payment then proceed to Provincial Treasurer’s Office (PTO) for payment
